Graduate Student Forms

On this page, you will find forms that you need to fill out to mark various watersheds in your graduate career.

As you will see, some forms are for our internal departmental records; these forms should be returned to Bernie Koprince, the graduate secretary, for your departmental file. Other forms are required by UT’s Graduate School and should be submitted according to the directions the Graduate School provides. You will find more information about Graduate School requirements here: http://gradschool.utk.edu/gradforms.shtml
