Graduate Student Forms

On this page, you will find forms that you need to fill out to mark various watersheds in your graduate career.

As you will see, some forms are for our internal departmental records; these forms should be returned to Bernie Koprince, the graduate secretary, for your departmental file. Other forms are required by UT’s Graduate School and should be submitted according to the directions the Graduate School provides. General Forms

  • Registration Form for Independent Study (History 521/ 593/ 621) – You will need to complete this form and submit it to Bernie Koprince before you will be able to register for History 521, History 593, or History 621. Once we have your paperwork, Mary Beckley will create a special section of the course for you in the computer system, and you will be able to register online.

 

MA Forms

  • Request for Concurrent Master’s Degree – If you were admitted as a PhD student and plan on getting a Concurrent MA while in the program, submitting this form is the first step in that process. After you submit this form to the Graduate School, you will then complete the rest of the paperwork to receive your MA, just as if you were a terminal MA student.
  • Graduate Student Information Form  – This form is for our internal departmental records; it should be submitted to the graduate secretary at the beginning of each academic year.
  • Declaration of Permanent Adviser Form – This form is for our internal departmental records; it should be submitted to the graduate secretary at the end of your first semester at UT.
  • MA Committee Form – This form is for our internal departmental records; it needs to be submitted to the graduate secretary by the end of your second semester at UT.
  • Admission to Candidacy — This form is for the Graduate School; consult their deadlines.
  • Graduation Application — This form is for the Graduate School; consult their deadlines.
  • Report of Final Exam/Defense of Thesis (pass/fail) — This form is for the Graduate School; consult their deadlines.
  • Thesis/Dissertation Approval Form – This form is for the Graduate School; consult their deadlines.
  • Alumni Information Form — Please submit this form to the graduate secretary before leaving UT.
  • If you decide to apply to continue on to the PhD program, you can find information about a Change of Program application on the Graduate School’s website: http://graduateadmissions.utk.edu/

 

PhD Forms

  • Request of Concurrent Master’s Degree –  If you plan on getting a concurrent MA on the way to completing your doctorate, submitting this form is the first step in that process.  After you submit this form to the Graduate School, you will then complete the rest of the paperwork to receive your MA, just as if you were a terminal MA student.
  • Graduate Student Information Form — This form is for our internal departmental records; it should be submitted to the graduate secretary at the beginning of each academic year.
  • Declaration of Permanent Adviser Form – This form is for our internal departmental records; it should be submitted to the graduate secretary by the end of the your first semester at UT.
  • Comprehensive Examination Committee Form – This form is for our internal departmental records; typically, it should be submitted to the graduate secretary approximately one year before you plan on taking the comprehensive exam.
  • Comprehensive Examination Grade Report – The adviser should complete this form after the comprehensive exam and return it to the graduate secretary.
  • Language Exam Form — Copies of this form will be provided to students when they sit for the History Department language exam.
  • Dissertation Prospectus Form — The dissertation prospectus should be circulated to the student’s dissertation committee and approved by the dissertation adviser before the end of the semester following completion of the comprehensive exam. Please submit this form and a copy of the dissertation prospectus to the graduate secretary for the department’s records.
  • Doctoral Committee Appointment — This form is for the Graduate School; consult their deadlines. Typically, students in our department will choose their doctoral committees soon after completing the comprehensive exam.
  • Admission to Candidacy — This form is for the Graduate School; consult their deadlines.
  • Graduation Application  — This form is for the Graduate School; consult their deadlines.
  • Scheduling Defense of Dissertation — This form is for the Graduate School; consult their deadlines.
  • Thesis/Dissertation Approval Form – This form is for the Graduate School; consult their deadlines.
  • Alumni Information Form — Please complete this form and turn it in to the graduate secretary before leaving UT.
